  • Task completion and drop-off points (privacy-safe analytics).
  • Accessibility regressions and usability bug rate.
  • Client error rate and navigational dead ends (404s).

Failure modes

  • Fragmented flows causing user confusion and support load.
  • Accessibility issues blocking critical actions.
  • Inconsistent terminology and IA drift.

Production readiness checklist
